A high-stakes EHR and billing transition
Wray Community District Hospital, a critical access provider in rural Colorado, faced a high-stakes operational challenge. It was transitioning from Cerner Millennium to Oracle Patient Accounting while simultaneously converting its Rural Health Clinic (RHC) billing structure to Method II.
Transition complexity and operational risk
This wasn’t a simple upgrade. The transition required coordinating multiple implementations at once and solving the issue that limitations in the existing system did not support Method II billing. At the same time, internal teams were managing day-to-day operations across a 15-bed Level IV trauma center, skilled nursing facility and assisted living services.
Leadership had clear concerns:

Workflow instability across billing and claims processes
Limited internal capacity to manage testing, configuration and issue resolution.
Without the right support, the risk was significant: delays in claims, reduced reimbursement and operational strain that could ultimately impact patient care.
Extensive testing and issue resolution
R1 partnered closely with Wray to help guide the organization through every phase of the transition. R1 brought technical expertise and operational support to ensure stability before, during and after go-live.
R1 runs almost 8 times more test scripts than Oracle provides. This helps ensure long-term success based on each client’s needs and system set up. R1 also makes sure all issues are resolved.
To help prepare Wray for a successful transition, R1 conducted extensive regression testing and test script execution tailored to Wray's system build, workflows and RHC Method II billing requirements. The team completed 306 total test scripts, including the 35 Oracle-provided scripts along with many additional scenarios designed to reflect Wray's operational needs.
This expanded testing approach led to identifying potential issues early and submitting configuration changes to prevent downstream disruptions. Working together, R1 and Wray also performed critical pre-transition cleanup, ensuring claims and workflows were optimized ahead of conversion.
Onsite go-live support
Recognizing the complexity of the transition, R1 deployed consultants onsite for key phases. This included two dedicated IT testing events for one week each and go-live support to stabilize operations in real time. This hands-on approach ensured that issues could be identified and resolved immediately, minimizing disruption.
Revenue cycle stabilization
R1 also helped with advocacy and collaboration while helping to preemptively resolve issues that could have slowed performance or delayed reimbursement. R1’s team extended beyond consulting, supporting critical functions such as claims processing and held claims resolution, AR follow-up and workflow stabilization, and training and process alignment for Wray staff.

As Wray geared up for the transition, R1 maintained consistent support in the legacy system during the wind-down period. This dedication allowed Wray to achieve 102% of their cash target and accelerate future cash performance.
Despite the complexity of the transition from Cerner Millennium to Oracle’s Patient Accounting system, Wray maintained stability and exceeded expectations across the following goals:
Surpass cash goals in the month following go-live
Maintain strong claims performance and avoid major disruptions
Complete a complex RHC Method II conversion
Clean claim rate improvement
With R1’s support, Wray was able to exceed its clean claim rate goal of 95% in the first week post go-live. Even when the clean claim rate dipped to 93% later, R1 stepped in to adjust builds and improve upstream workflows to get back to the goal.

The transition is widely recognized as a success. Oracle noted this was the smoothest transition it had seen, underscoring the value of having an experienced partner guiding the process.
